    Abstract [en]

    The purpose of this study is to identify which treatment models that are available for men who use violence against women in intimate relationships and the effects of the most commonly used treatment interventions. The further aim is to examine the theoretical assumptions and processing content of treatment models. The present study was conducted as a literature review where knowledge about treatment models for violent men in intimate relationships are compiled. Our results demonstrate that the treatment models that are most commonly used are Emerge, The Duluth Model, Change and ATV (Alternatives to Violence). Evaluations show that most treatment models exhibit small but significant effects and recurrence of violent crime decreases in the short term. Research highlights, however problems about the difficulties associated with evaluating treatment programs.

    Abstract [sv]

    Bakgrund: Sepsis är ett allvarligt tillstånd som är vanligt förekommande och medför en hög riskför mortalitet. Trots den vanliga förekomsten råder en stor okunnighet om tillståndet. I många århar olika markörer på akutmottagningen studerats, för att ta reda på vilka markörer som äreffektivast på att förutsäga en patients sjukdomsutveckling och utfall. Det finns än idag en storoenighet bland forskare om vilka markörer som är bäst. Laktat, andningsfrekvens (RR;respiratory rate) och base excess (BE) har använts som prognostiska markörer tidigare, menvilken av dem som är bäst är okänt. För behandling av sepsis finns det riktlinjer som säger attantibiotika bör administreras inom en timme efter ankomst till akutmottagningen. Det har visatsig vara avgörande för prognosen hur snabbt antibiotika sätts in.

    Syfte: Att jämföra det prognostiska värdet hos laktat med respirationsfrekvens (RR) med Baseexcess(BE) i blodet, samt att undersöka hur väl de nationella riktlinjerna för initial behandling avsepsis har följts och vilken inverkan på patientutfall detta har.

    Metod: En retrospektiv journalgranskningsstudie gjordes på alla patienter som har vårdats försepsis på IVA på Universitetssjukhuset Örebro under perioden juli 2017 – juli 2018. Utav totalt 62patienter som selekterades ur ett sepsisregister kunde 50 stycken inkluderas i undersökningen.Studiepopulationen indelades i två grupper: mortalitet inom 30 dagar eller ej.

    Resultat: Av de inkluderade 50 patienterna avled 12 stycken (24%) och 38 stycken (76%)utvecklade septisk chock, 42 stycken (88 %) fick vätska administrerat inom 1 h och 26 stycken (52%) fick antibiotika inom 1 h. Mann Whitney U-test visade att fördelningen av laktatvärden (p =0,166), andningsfrekvensvärden (p = 0,059) och base excess-nivåer (p = 0,077) hos gruppen meddödsfall inom 30 dagar jämfört med gruppen utan dödsfall inom 30 dagar inte var statistisktsignifikanta. Log rank test visade: laktat > 4 mmol/l (p = 0,136), RR ≥ 30/min (p = 0,037), BE < -3mmol/l (p = 0,802), antibiotika administrerat inom 1 h (p = 0,636).

    Slutsats: Andningsfrekvens var en bättre prognostisk markör på akutmottagningen än laktat ochbase excess. En otillfredsställande låg andel av patienterna fick antibiotika inom en timme efterankomst till akutmottagningen. Administrering av antibiotika inom en timme efter ankomst tillakutmottagning visade sig inte vara avgörande för prognosen.

    Abstract [en]

    The growth of social media has set demands on organizations to be online and interact with

    stakeholders, especially during a crisis. The public are no longer seen as passive receivers of

    marketing messages. Previous studies have shown that the need for information increases

    during a crisis. Social media can be a powerful tool if is used strategically. This case study

    looks deeper into Oxfam’s 2018 Haiti sexual exploitation scandal, as an attempt to understand

    how organizations communicate on social media during a crisis. By co-applying multimodal

    critical discourse analysis (MCDA) and the social-mediated crisis communication (SMCC)

    model, a broader understanding of how the crisis was handled can be developed. The data

    consists of four Instagram posts that will be analyzed, drawing upon four multimodal

    frameworks from Machin (2017): Iconography: the ‘hidden meanings’ of images; The meaning

    of color in visual design; The meaning of typography; and Representation of social actors in

    images. In order to obtain a broader picture of the strategies, key public and relationships, the

    components of the SMCC model will be identified and presented for this case. The result of

    this study shows that multiple response strategies have been used to communicate both tailored

    messages and unified organizational messages. It is apparent that Oxfam did not have a clear

    strategy and altered between apologizing, “blaming” individuals within the organization and

    distancing themselves from the crisis.

    I den här är fallstudien har vi valt att undersöka hur två organisationer som arbetar med klimatomställning använder storytelling. Den ena organisationen, EU, har drivit projektet Leader sedan 1990-talet, den andra är Svenska Naturskyddsföreningen som 2013 skapade en julkalender för sin hemsida. I studien undersöks sambandet mellan organisationernas bild av engagemang och berättelsernas struktur. Studien utgår från två teoretiska perspektiv Governmentality och narrativ teori. Sökningen av berättelserna har skett genom ett målstyrt urval och två kvalitativa metoder har tillämpats: text- och narrationsanalys.

    Introduction: The incidence of hyperthyroidism has been reported in various countries to be 23-93/100000 inhabitants per year. This extended study has evaluated the incidence for similar to 40% of the Swedish population of 9 million inhabitants. Sweden is considered to be iodine sufficient country. Methods:All patients including children, who were newly diagnosed with overt hyperthyroidism in the years 2003-2005, were prospectively registered in a multicenter study. The inclusion criteria are as follows:clinical symptoms and/or signs of hyperthyroidism with plasma TSH concentration below 0.2 mIE/l and increased plasma levels of free/total triiodothyronine and/or free/total thyroxine. Patients with relapse of hyperthyroidism or thyroiditis were not included. The diagnosis of Graves' disease (GD), toxic multinodular goiter (TMNG) and solitary toxic adenoma (STA), smoking, initial treatment, occurrence of thyroid-associated eye symptoms/signs, and demographic data were registered. Results:A total of 2916 patients were diagnosed with de novo hyperthyroidism showing the total incidence of 27.6/100 000 inhabitants per year. The incidence of GD was 21.0/100 000 and toxic nodular goiter (TNG=STA+TMNG) occurred in 692 patients, corresponding to an annual incidence of 6.5/100 000. The incidence was higher in women compared with men (4.2:1). Seventy-five percent of the patients were diagnosed with GD, in whom thyroid-associated eye symptoms/signs occurred during diagnosis in every fifth patient. Geographical differences were observed. Conclusion:The incidence of hyperthyroidism in Sweden is in a lower range compared with international reports. Seventy-five percent of patients with hyperthyroidism had GD and 20% of them had thyroid-associated eye symptoms/signs during diagnosis. The observed geographical differences require further studies.

    Förskolan är en verksamhet som enligt förskolans läroplan ska utformas efter varje barns behov. Det innebär att mängden stöd olika barn behöver kan variera, och vissa barn kan behöva särskilt stöd för att utifrån sina förutsättningar kunna utvecklas så långt som möjligt. Forskning visar dock att det finns en brist på kunskap om vilka som ska definieras som i behov av särskilt stöd. Det är heller inte känt vilka kriterier pedagogerna använder för att ett barn ska få särskilt stöd. Samtidigt kan vilket stöd ett barn ges komma att bli en avgörande faktor för barnets både utveckling och lärande, vilket belyser en problematik. Utifrån detta är studiens syfte att undersöka pedagogers uppfattning av gränsdragningen mellan barn som behöver och inte behöver särskilt stöd. Detta gjordes genom kvalitativa intervjuer med pedagoger i förskolans verksamhet. Resultaten som framträdde var hur gränsdragningen är en komplex praktik med påverkan från ett flertal olika faktorer, och därför svår för pedagogerna att handskas med. Detta har vi sedan försökt förstå genom teorin socialkonstruktivism, vilket medfört att gränsdragningen då uppfattas som en social konstruktion påverkade av andra sociala konstruktion och något “rätt” kring gränsdragningen blir svår att nå.

    "Det sociala" tillhör de begrepp som samhällsvetenskapen utgår från men sällan känner något behov av att förklara. Ofta är socialiteten den samhällsvetenskapliga praktikens utgångspunkt och mål, men dess innebörd kan inte fastställas utan att överskrida gränserna till t.ex. filosofi, historia och religion. Kanske är avsaknaden av en kontinuerlig reflektion över det socialas innebörd rent av ett försvar av det samhällsvetenskapliga fältet. I denna bok diskuterar tio samhällsvetare och humanister det socialas betydelse från Immanuel Kant till Slavoj Zizek. Det handlar om tio sätt att tänka kring det sociala, om tio sociologiker. Detta skulle kunna tolkas som att det sociala är det objekt bokens olika bidrag behandlar ur respektive perspektiv. Men det kan också ges en snävare tolkning. Det verkliga objektet för undersökningarna blir då själva relationen mellan det sociala och vetandet. Med detta synsätt kan det sociala inte uppfattas som ett neutralt eller autonomt faktum utanför tänkandet. Snarare blir socialiteten något som framträder först i vetandets konkreta gränsdragningar och skillnadsskapanden. Denna bok är ett mångbottnat bidrag till diskussionen om vad samhället är och om möjligheterna att förändra det.

    Abstract [en]

    Purpose: This paper aims to examine the mobilization of management accounting (MA) numbers and metrics in social interactions. The purpose is to develop a model of how and why managers perceive and mobilize (new) MA numbers/metrics in a changing way over time in situated face-to-face interactions.

    Design/methodology/approach: An observation-based qualitative field study of a change project in a large manufacturing company is used as the basis for our analysis.

    Findings: The empirical study shows that MA numbers and metrics are essential when semi-distant managers strive to solve problems and achieve radical improvement targets, but that the ways in which existing and new metrics are perceived and mobilized during face-to-face interactions change over time. The study provides both a detailed account of the emergent nature of the transformation process and a number of mechanisms as to why managers (inter-)act the way they do to produce such change.

    Originality/value: The paper problematizes the generally held view that MA numbers and metrics primarily work as a structuring device in face-to-face interactions, and also, how the processes are constituted through which MA is transformed into such a structuring device. The paper also adds new insights to our understandings of why managers (inter-)act the way they do to produce MA change.

    Abstract [en]

    Purpose – This paper aims to examine how and why management accounting practices are linked to an organization's identity and identity discrepancies.

    Design/methodology/approach – A qualitative field study of a one-year change project in a large manufacturing company is used as the basis for the analysis.

    Findings – The empirical study reveals how discrepancies between organizational members' perceived identity and their construed external (and desired future) image both influence and are influenced by emergent accounting practices. Empirical evidence suggests such a reciprocal relationship between accounting and identity, since accounting practices are an important means of (de)legitimizing an organization's current self-perception.

    Research limitations/implications – The uncovered reciprocal relationship between management accounting practices and organizational identity (discrepancies) have implications for a broader literature, including the works on how different forms of control interact as a “control package” and the discourse on potential sources of organizational identity change.

    Originality/value – Although it has previously been suggested that management accounting may be an important means for, as well as an outcome of, processes of identity (re)constructions in organizations, this study suggests a more complex interplay than has previously been noted in the literature. Specifically, it was found that organizational identity may for a considerable time work as a highly influential and largely unquestioned categorical imperative, signifying the boundaries of appropriate organizational action. At times, however, accounting practices may spark (re)constructions of identity discrepancies through: providing identity-inconsistent evidence; and using (new) measures in a “feed-forward” manner to explore possible ways to close such perceived discrepancies.

    Abstract [en]

    Purpose: Based on an institutional perspective, this study explores the role of management accounting (MA) in promoting or impeding changes in the employees’ conceptions of shopfloor worker responsibility in a company trying to implement a Continuous Improvement (CI) working practice.

    Methodology/approach: We use an ethnographically inspired research method where weekly CI meetings in two workgroups were observed over a period of eight months and in-depth interviews with managers and operators were conducted regularly.

    Findings: The study reveals that active and skilful exploiters of inconsistencies within social arrangements may use MA as one important way of transforming a traditional vertical view of worker responsibility into a more horizontally-oriented view by: creating collective reflection and reasoned analysis of the limits of the present order; and, by visualizing and justifying an alternative model(s) of social behaviour. However, the study also shows that MA may contribute to the reinforcement of a vertical view by the use of group-level measures strictly as a one-way performance monitoring device.

    Research limitations/implications: Arguably, it is worthwhile to explore the existence of ‘institutional heterogeneity’ because our study highlights that ‘contradictions’ between social orders may not only nurture institutional stability, but may also be a necessary (although not sufficient) condition for institutional change.

    Abstract [en]

    The two studies reported in this thesis are about problem solving at a low organisational level in an organisational unit in a Swedish manufacturing company, which has adopted ideas of World Class Manufacturing. In the first study, we analyse, from an organisational learning perspective, the implementation of the well-known concept Continuous Improvement at shop-floor level and how management accounting may facilitate or impede improvement work. A case study was conducted in two workgroups in one production unit for about seven months. In one of the groups, management accounting facilitated improvement work with high organisational scope and, in the other group, impeded the organisational scope. Our conclusion is that management accounting may facilitate a high organisational scope by supporting the leadership style, stressing co-operation, dialog and participation, or, by the way the manager uses management accounting, make boundaries distinct between groups of employees and between departments, and therefore impede improvements with high organisational scope. In the second study, we analyse, from a rule-based behavioural perspective, how ideas of World Class Manufacturing are institutionalised in cross-functional problem solving at a low organisational level. A case study was carried out in two cross-functional weekly group meetings for about nine months. Problems raised and solutions decided followed a repeated pattern. The actors draw on instrumental rules, which can be described as technical as well as economic. We concluded that ideas of World Class Manufacturing were institutionalised as different variants of World Class Manufacturing in the different functions at a low organisational level. At the meetings, contradictory instrumental rules were activated by the functions and treated according to interaction rules. Two overall patterns of action, articulated the two main interaction rules guiding actors in how to treat contradictory ideas of World Class Manufacturing.

    Uppsatsen handlar om den åldrande befolkningen som är ett internationellt fenomen och en viktig samhällsutmaning. Fokus i uppsatsen ligger på nationell och framför allt kommunal nivå. Syftet är att redogöra för konsekvenserna av en åldrande befolkning och att redogöra för de åtgärder som finns på nationell nivå och framför allt på kommunal nivå genom att undersöka fyra utvalda kommuner (Örebro, Kumla, Hällefors och Ljusnarsberg). Syftet är även att kartlägga andelen äldre i svenska kommuner. Både kvantitativ och kvalitativ metod har använts. Det kvantitativa materialet utgörs av offentlig statistik och det kvalitativa materialet utgörs av åtta intervjuer, två intervjuer för varje kommun.

    Abstract [en]

    What kind of norms and values determine a good friendship? In aqualitative study three men and three women aged 20-30 years oldwere interviewed. The respondents who were recruited through aconvenience sampling were asked to answer questions concerningtheir view about friendship. After analyzing the interview protocol,three common themes were identified: trust acceptance and intimacy.Friendship showed to be a significant part when it came to people andtheir wellbeing. The study also showed that men and women haddifferent expectations for what that friendship will entail and thesedifferences were based on norms and values. It seems that norms andvalues are developed through the existing culture in society and inover time.

    Abstract

    The aim with this study is to investigate young adult´s experiences and need of social work in school, and a qualitative method was used to take part of their experiences. Groupquestionnaires with open questions was formed and three year one classes in uppersecondary school participated. In the process of making the questions a testquestionnaire was tested with one of the three classes as particpants. Information has also been collected throug an interview over the phone with the school´s welfare officer, and the school´s policy dokuments have been read. Social work in schools is mentioned in the study as preventive measures with youths when it comes to bullying, alcohol, drugs, sex and coexistance and tobacco. The concept youths include individuals born in 1989 and the year they was born was chosen in the study instead of a specific age to include youths who are not yet sixteen and to exclude the ones who are born earlier. The main questions are: What kind of experiences do yoths have of social work in school? What kind of need do youths have of social work in school? The result show the respondents experiences and needs of social work in school throug quotation and the answers. The result is also illustrated throug two diagrams which show the questionnaires frequence of answer. There is a need of social work in the uppersecondary school, and the need of it is not more then what the school today can offer. The respondents had the oppotunity to freely be able to express their opinion of social work in school, but the result shows that the youths who participated do not have the interest to give their opinion in the subject. This emerge throug answers with few words and a high frequence of “don´t know”-answers in many of the questions in the questionnaire. The analysis and discussion in this study opens up for new questions that can be used as further research. Key words: social work in school, youths

    Background Low total diversity of the gut microbiota during the first year of life is associated with allergic diseases in infancy, but little is known how early microbial diversity is related to allergic disease later in school age. Objective To assess microbial diversity and characterize the dominant bacteria in stool during the first year of life in relation to the prevalence of different allergic diseases in school age, such as asthma, allergic rhinoconjunctivitis (ARC) and eczema. Methods The microbial diversity and composition was analysed with barcoded 16S rDNA 454 pyrosequencing in stool samples at 1week, 1month and 12months of age in 47 infants which were subsequently assessed for allergic disease and skin prick test reactivity at 7years of age (ClinicalTrials.gov ID NCT01285830). Results Children developing asthma (n=8) had a lower diversity of the total microbiota than non-asthmatic children at 1week (P=0.04) and 1month (P=0.003) of age, whereas allergic rhinoconjunctivitis (n=13), eczema (n=12) and positive skin prick reactivity (n=14) at 7years of age did not associate with the gut microbiota diversity. Neither was asthma associated with the microbiota composition later in infancy (at 12months). Children having IgE-associated eczema in infancy and subsequently developing asthma had lower microbial diversity than those that did not. There were no significant differences, however, in relative abundance of bacterial phyla and genera between children with or without allergic disease. Conclusion and Clinical Relevance Low total diversity of the gut microbiota during the first month of life was associated with asthma but not ARC in children at 7years of age. Measures affecting microbial colonization of the infant during the first month of life may impact asthma development in childhood.

    Background: It is debated whether a low total diversity of the gut microbiota in early childhood is more important than an altered prevalence of particular bacterial species for the increasing incidence of allergic disease. The advent of powerful, cultivation-free molecular methods makes it possible to characterize the total microbiome down to the genus level in large cohorts.

    Objective: We sought to assess microbial diversity and characterize the dominant bacteria in stool during the first year of life in relation to atopic eczema development.

    Methods: Microbial diversity and composition were analyzed with barcoded 16S rDNA 454-pyrosequencing in stool samples at 1 week, 1 month, and 12 months of age in 20 infants with IgE-associated eczema and 20 infants without any allergic manifestation until 2 years of age (ClinicalTrials.gov ID NCT01285830).

    Results: Infants with IgE-associated eczema had a lower diversity of the total microbiota at 1 month (P = .004) and a lower diversity of the bacterial phylum Bacteroidetes and the genus Bacteroides at 1 month (P = .02 and P = .01) and the phylum Proteobacteria at 12 months of age (P = .02). The microbiota was less uniform at 1 month than at 12 months of age, with a high interindividual variability. At 12 months, when the microbiota had stabilized, Proteobacteria, comprising gram-negative organisms, were more abundant in infants without allergic manifestation (Empirical Analysis of Digital Gene Expression in R [edgeR] test: P = .008, q = 0.02).

    Conclusion: Low intestinal microbial diversity during the first month of life was associated with subsequent atopic eczema.

    Background: Supplementation with the probioticLactobacillus reuteri reduced the incidence of IgE-associated allergic disease in infancy. This treatment might therefore also reduce the risk of asthma and allergic rhinoconjunctivitis in school age.

    Objective: To evaluate whether perinatal and infant supplementation withL.reuteri reduced the prevalence of respiratory allergic disease in school age and to explore whether this supplementation was associated with any long-term side effects.

    Methods: A randomized, placebo-controlled trial with oral supplementation withL.reuteriATCC 55730 (1x10(8)CFU) during the last month of gestation and through the first year of life comprising 232 families with allergic disease, of whom 184 completed a 7-yr follow-up. The primary outcomes at 7yr of age were allergic disease and skin prick test reactivity (ClinicalTrials.govID NCT01285830).

    Results: The prevalence of asthma (15% in the probiotic vs. 16% in placebo group), allergic rhinoconjunctivitis (27% vs. 20%), eczema (21% vs. 19%) and skin prick test reactivity (29% vs. 26%) was similar in the probiotic and placebo group. Growth indices and gastrointestinal symptoms were similar in the two groups. No severe adverse events were reported.

    Conclusion: The effect ofL.reuteri on sensitization andIgE-associated eczema in infancy did not lead to a lower prevalence of respiratory allergic disease in school age. Thus, the effect ofL.reuteri on the immune system seems to be transient. Administration ofL.reuteri during the last weeks of gestation and in infancy was not associated with any long-term side effects.

    Abstract [en]

    This paper investigates the sensitivity of Colombian GDP growth to the surrounding macroeconomic environment. We estimate a Bayesian VAR model with informative steady-state priors for the Colombian economy using quarterly data from 1995 to 2007. A variance decomposition shows that world GDP growth and government spending are the most important factors, explaining roughly 17 and 16 per cent of the variance in Colombian GDP growth respectively. The model, which is shown to forecast well out-of-sample, can also be used to analyse alternative scenarios. Generating both endogenous and conditional forecasts, we show that the impact on Colombian GDP growth of a substantial downturn in world GDP growth would be non-negligible but that the decline still would be mild by historical standards.

    Abstract [en]

    We have previously shown that gadolinium oxide (Gd2O3) nanoparticles are promising candidates to be used as contrast agents in magnetic resonance (MR) imaging applications. In this study, these nanoparticles were investigated in a cellular system, as possible probes for visualization and targeting intended for bioimaging applications. We evaluated the impact of the presence of Gd2O3 nanoparticles on the production of reactive oxygen species (ROS) from human neutrophils, by means of luminol-dependent chemiluminescence. Three sets of Gd2O3 nanoparticles were studied, i.e. as synthesized, dialyzed and both PEG-functionalized and dialyzed Gd2O3 nanoparticles. In addition, neutrophil morphology was evaluated by fluorescent staining of the actin cytoskeleton and fluorescence microscopy. We show that surface modification of these nanoparticles with polyethylene glycol (PEG) is essential in order to increase their biocompatibility. We observed that the as synthesized nanoparticles markedly decreased the ROS production from neutrophils challenged with prey (opsonized yeast particles) compared to controls without nanoparticles. After functionalization and dialysis, more moderate inhibitory effects were observed at a corresponding concentration of gadolinium. At lower gadolinium concentration the response was similar to that of the control cells. We suggest that the diethylene glycol (DEG) present in the as synthesized nanoparticle preparation is responsible for the inhibitory effects on the neutrophil oxidative burst. Indeed, in the present study we also show that even a low concentration of DEG, 0.3%, severely inhibits neutrophil function. In summary, the low cellular response upon PEG-functionalized Gd2O3 nanoparticle exposure indicates that these nanoparticles are promising candidates for MR-imaging purposes.
