    Sjuk av självförverkligande: individen i det nya arbetslivet2011Bok (Annet vitenskapelig)
    Abstract [sv]

    Sedan 1990-talet har antalet anställda inom svenskt arbetsliv som drabbats av psykiska besvär på grund av stress mer än fördubblats. Den här studien belyser utbrändas erfarenheter av sin identitet och psykosociala arbetsmiljö med utgångspunkt från de senaste årtiondenas djupgående och strukturella samhälleliga omvandlingar. Avsikten är att finna helhetsförklaringar på samhällsnivå, framförallt vilka konsekvenser tendenserna till ökad individualisering i samhället med nya och höjda krav får för arbetstagares psykiska hälsa.

    Undersökningen bygger på djupintervjuer med långtidssjukskrivna för olika utmattningsdiagnoser. Materialet består även av bloggar och andra texter av de intervjuade. Analysen sker med hjälp av Axel Honneths transformationsteoretiska tanke att dagens arbetsliv utmärks av institutionaliserade förväntningar på självförverkligande. Därmed kan studien presentera en samhällsorienterad förklaringsmodell som flyttar fokus från den sjuke till stressrelaterade symptoms samhälleliga orsaker genom att betrakta dem som vår tids samhällsjukdom, något som hittills i hög utsträckning saknats inom svensk stressforskning.

    Resultaten visar att tiden före sjukskrivningen strävade samtliga intervjupersoner efter att vara autentiska genom att utforska och visa upp sina förmågor och egenskaper i arbetet. Mer eller mindre omedvetet följde deras uttryckande av sig själva vissa standardiserade mönster. Gemensamma nämnare är deras engagemang, initiativrikedom, framåtanda, flexibilitet och utvecklingsvillighet. Gemensamt för studiens deltagare är även att deras arbetsplats varit eller var utsatt för omfattande omstruktureringar och/eller nedskärningar vilka de haft litet inflytande över. I kölvattnet följde bland annat konflikter, splittrade arbetsgrupper, ökad arbetsbelastning, ökade och nya krav, oklara och motsägelsefulla förväntningar och oklar ansvarsfördelning. Av undersökningen framgår att sökandet efter sig själv på en arbetsplats präglad av sådana följdverkningar blev den utlösande faktorn bakom samtligas utveckling av utmattningssymptom.

    De intervjuades självkänsla var nära förbunden med att verka för altruistiska värden, som på olika sätt dikterar omsorg om andra. Med önskan om att vara sanna mot sig själva och sina värden tog alla personligt ansvar för bristerna i organisation och ledning. Den gränslösa arbetssituationen gav till slut upphov till en ond spiral av värdelöshetskänslor och utmattning. En bidragande orsak till den bristande självkänslan var upplevelsen av en konflikt mellan att vara autentisk på egna villkor och att uttrycka och utveckla sin person på ett villkorat sätt. Med hjälp av Honneths terminologi förstår jag detta som att personerna utvecklade en social patologi som en reaktion på arbetsvillkoren, framförallt upplevelsen av otillräcklig gemensam mening skapad av människor i deras arbetsvardag.

    I samband med sjukskrivningen var det vanligt att indirekt börja ifrågasätta den standardiserade mallen för självförverkligande. En mer medveten strävan efter självförverkligande för sin egen skull och på egna villkor inleddes av intervjupersonerna. Detta nya självförverkligandeprojekt hamnade lätt i konflikt med det gränslösa arbetet, vilket komplicerade återvändandet till arbetslivet.

    Resultaten pekar på faktorer utanför individen som kan åtgärdas för att förebygga utbrändhet. Några av studiens slutsatser är att tydliga strukturer för beslutsfattande, gemensamt uppsatta mål och visioner samt delaktighet är avgörande för ett ovillkorat självförverkligande.

    Abstract [en]

    Background and purpse: Previous national epidemiological data on the characteristics and trends of patients with ankle fractures have been limited. We therefore analyzed data on Swedish inpatients with ankle fractures in this nationwide population study, based on data from 1987 through 2004.

    Patients and methods: Data on all inpatients aged 15 years and older with ankle fracture were extracted from the Swedish National Patient Register for the period 1987-2004.

    Results: We identified 91,410 hospital admissions with ankle fracture, corresponding to an annual incidence rate of 71 per 10(5) person-years. During the study period, the number of hospital admissions increased by 0.2% annually, mainly from increase in fracture incidence in the elderly women. Mean age at admission was 45 (SD 19) years for men and 58 (18) for women. The major mechanism of injury was falling at the same level (64%).

    Interpretation: This nationwide study of inpatients with ankle fractures showed an increase in fracture incidence, particularly in elderly women.

    In some cost benefit analysis (CBA) applications, such as those used for the valuation of climate change damage, distributional weights are used to account for diminishing utility of marginal income. This is usually done by means of intra-temporal distributional weights, which are combined with discounting to account for inter-temporal equity and efficiency. Here, I show that this approach might introduce some inconsistencies in terms of path dependence. In short, this inconsistency means that regional economic growth is double counted. This is because income weighting is performed both through the discount rate and through the distributional weights such that growth shows up twice in the weighting process. Using the PAGE2002 model, it is found that the inconsistency problem in the original model erases the influence of distributional weights on the social cost of carbon dioxide (SCCO2) compared to a standard CBA approach. The alternative approaches proposed here yield about 20%–40% higher values of SCCO2 than the old approach. While this has been briefly commented on in previous work, it has not yet been more thoroughly analyzed nor communicated to the broader community of climate policy and economic analysts who are not deeply interested in the specifications of the climate impact assessment models.

    Abstract [en]

    This compilation dissertation consists of four essays with the common theme of welfare analysis of long-term public investments. The first two essays focus on analysis of climate change mitigation, i.e., the social cost of carbon dioxide. The third essay focuses on cost-benefit analysis (CBA) of transport investment projects, while the last essay takes a broader perspective on welfare analysis.

    Essay 1: The Temporal Aspects of the Social Cost of Greenhouse Gases. The purpose of Essay 1 is to investigate the temporal aspects of the social cost of greenhouse gases. I find that the calculation period should ultimately be modeled to be consistent with the discount rate and that the “global-warming potential” concept is unsuitable for calculation of the social cost of GHGs other than carbon dioxide.

    Essay 2: Avoiding path dependence of distributional weights: Lessons from climate change economic assessments. In Essay 2, I explore shortcomings in income weighting in evaluation of climate change policy. In short, in previous versions of two of the most important existing models, regional economic growth is double counted. The proposed alternative approaches yield about 20–40% higher values of SCCO2 than the old approach.

    Essay 3: Does uncertainty make cost-benefit analyses pointless? In Essay 3, the aim is to investigate to what extent CBA improves the selection decision of projects when uncertainties are taken into account, using a simulation-based approach on real data of infrastructure investments. The results indicate that, in line with previous literature, CBA is a rather robust tool and considerably increases the quality of decision making compared with a random selection mechanism, even when high levels of uncertainty are considered.

    Essay 4: Household Production and the Elasticity of Marginal Utility of Consumption. In Essay 4, I develop a new model to show that omission of household production in a previous model leads to bias when the elasticity of marginal utility of consumption, EMUC, is estimated. I further offer new, unbiased estimates based on current evidence of the included parameters, suggesting a lower bound of EMUC at about 0.9.

    Abstract [en]

    Chetty (2006) developed a new method of estimating the Elasticity of Marginal Utility of Consumption (EMUC) from observed work time responses to wage changes and derived an upper bound of 2 for this parameter. Here I show that the omission of household production in Chetty’s model may lead to bias, and perform a numerical sensitivity analysis of Chetty’s results in this respect. I develop a new model that includes household production from which I derive new, unbiased EMUC formulas. I offer empirical estimates based on current evidence of the included parameters, suggesting a lower bound for EMUC of about 0.9.

    The purpose of this study is to investigate the temporal aspects of the social cost of greenhouse gases (GHGs). I am particularly interested in the interaction between time of emissions, discounting, and type of GHG (where different GHGs have different atmospheric lifetimes). I show graphically how the social costs of GHGs depend on different parameter values and how the global damage potential for methane and sulfur hexafluoride evolves over time. I find that that the calculation period ultimately should be modeled to be consistent with the discount rate and that the "global-warming potential" concept is unsuitable for calculating of the social cost of GHGs other than carbon dioxide.

    Abstract [en]

    Cost-benefit analysis (CBA) is widely used in public decision making on infrastructure investments. However, the demand forecasts, cost estimates, benefit valuations and effect assessments that are conducted as part of CBAs are all subject to various degrees of uncertainty. The question is to what extent CBAs, given such uncertainties, are still useful as a way to prioritize between infrastructure investments, or put differently, how robust the policy conclusions of CBA are with respect to uncertainties. Using simulations based on real data on national infrastructure plans in Sweden and Norway, we study how investment selection and total realized benefits change when decisions are based on CBA assessments subject to several different types of uncertainty. Our results indicate that realized benefits and investment selection are surprisingly insensitive to all studied types of uncertainty, even for high levels of uncertainty. The two types of uncertainty that affect results the most are uncertainties about investment cost and transport demand. Reducing uncertainty can still be worthwhile, however, because of the huge amounts of money at stake: a 10% reduction in general uncertainty can increase the realized benefits of a national infrastructure investment plan by nearly 100 million euro (assuming that decisions are based on the CBAs). We conclude that, despite the many types of uncertainties, CBA is able to fairly consistently separate the wheat from the chaff and hence contribute to substantially improved infrastructure decisions.

    Abstract [en]

    This thesis aims to describe the initial phase of an acute coronary syndrome (ACS) in overall terms from a national perspective and to evaluate the impact of an information campaign designed to inform the public about how to act when suspecting an ACS. A total of 1939 patients at 11 hospitals in Swedenwith diagnosed ACS and symptom onset outside hospital completed a questionnaire(I-IV).In Study V, a questionnaire was completed by 116 patients withACS before the campaign and 122 after it. Register data were followed every year to evaluate ambulance use and emergency department (ED) visits.

    With regard to symptoms, patients with ST-elevation ACS (STE-ACS) more frequently had associated symptoms and pain with an abrupt onset reaching maximum intensity within minutes. However, fewer than half the patients with STE-ACS had this type of symptom onset. There were more similarities than differences between genders and differences between age groups were minor (I).

    Three-quarters of the patients interpreted the symptoms as cardiac in origin. The majority contacted a family member after symptom onset, whereas few called directly for an ambulance. Approaching someone after symptom onset and the belief that the symptoms were cardiac in origin were factors associated with a shorter pre-hospital delay (II).

    Half the patients went to hospital by ambulance. Independent factors for ambulance use were knowledge of the importance of quickly seeking medical care and calling for an ambulance when experiencing chest pain, severe symptoms, abrupt onset of pain, STE-ACS, increasing age and distance to hospital of > 5 km. Reasons for not calling for an ambulance were thinking self-transport would be faster or not being ill enough (III). Pain with abrupt onset, STE-ACS, symptoms such as vertigo or near syncope, experiencing the pain as frightening, interpreting the pain as cardiac in origin and knowledge were major factors associated with a short delay between symptom onset and decision to seek medical care, patient decision time (IV).

    The information campaign did not result in a reduction in patient decision time, but it appeared to increase ambulance use and the number of patients seeking the ED for acute chest pain (V).

    BACKGROUND:

    Various efforts to reduce patient delay in acute coronary syndrome (ACS) have had limited success. One reason might be a misinterpretation of the symptoms of ACS. The aim of this study was therefore to explore the characteristics and severity of symptoms among patients with an ACS in overall terms and in relation to the type of ACS, sex, age, and diabetes.

    METHODS:

    A total of 1939 patients at 11 hospitals in Sweden answered a questionnaire containing questions relating to the localization and intensity of symptoms, the presence of associated symptoms, the characteristics and experience of pain/symptoms, and the type of symptom onset.

    RESULTS:

    Patients with ST elevation differed from those without by more frequently having associated symptoms. They had higher pain/discomfort intensity and more frequently had pain with abrupt onset reaching maximum intensity within minutes. However, this type of symptom onset was only seen in less than half the patients with ST elevation and only 1 in 5 fulfilled all the criteria usually associated with a severe heart attack. Women differed from men in a few respects. They more frequently reported pain/discomfort in the neck or jaw and back, vomiting, and scored their pain/discomfort slightly higher than men. Differences between age groups were minor and there was no difference between patients with and without diabetes.

    CONCLUSIONS:

    The most striking finding was the low proportion of patients with the type of symptoms that are commonly associated with ACS. This is important for the planning of educational campaigns/programs to reduce patient delay.

    Objective The objective was to study patients’ interpretations, thoughts, and actions after symptom onset in acute coronary syndrome (ACS) in total and in relation to gender, age, history of coronary artery disease, type of syndrome, and residential area and its influence on prehospital delay. Setting We performed a national survey comprising intensive cardiac care units at 11 hospitals in Sweden. Method A total of 1939 patients with diagnosed ACS and symptom onset outside hospital completed a questionnaire containing standardized questions within 3 days after admission. Results Three-quarters of the patients interpreted their symptoms as cardiac in origin, and the most common reason was that they knew someone who had had an acute myocardial infarction. The majority contacted a family member, whereas only 3% directly called for an ambulance. Interpreting the symptoms as cardiac in origin and severe pain were major reasons for deciding to seek medical care. Approaching someone after symptom onset and the belief that the symptoms were cardiac in origin were factors associated with a shorter prehospital delay, whereas taking medication to relieve pain resulted in the opposite. The reaction pattern was influenced by gender, age, a history of coronary artery disease, and the type of ACS, but to a lesser extent by residential area. Conclusions Interpreting symptoms as cardiac in origin and approaching someone after symptom onset were major reasons for a shorter prehospital delay in ACS.

    Abstract [en]

    Background

    National guidelines recommend activation of the emergency medical service by patients who have symptoms of acute coronary syndrome (ACS). In spite of this, only 50% to 60% of persons with myocardial infarction initiate care by using the emergency medical service. The aim of this study was to define factors influencing the use of ambulance in ACS.

    Methods

    The method used in this study was a national survey comprising intensive cardiac care units at 11 hospitals in Sweden; 1,939 patients with diagnosed ACS and symptom onset outside the hospital completed a questionnaire a few days after admission.

    Results

    Half of the patients went to the hospital by ambulance. Factors associated with ambulance use were knowledge of the importance of quickly seeking medical care and calling for an ambulance when having chest pain (odds ratio [OR] 3.61, 95% CI 2.43-5.45), abrupt onset of pain reaching maximum intensity within minutes (OR 2.08, 1.62-2.69), nausea or cold sweat (OR 2.02, 1.54-2.65), vertigo or near syncope (OR 1.63, 1.21-2.20), ST-elevation ACS (OR 1.58, 1.21-2.06), increasing age (per year) (OR 1.03, 1.02-1.04), previous history of heart failure (OR 2.48, 1.47-4.26), and distance to the hospital of >5 km (OR 2.0, 1.55-2.59). Those who did not call for an ambulance thought self-transport would be faster or did not believe they were sick enough.

    Conclusions

    Symptoms, patient characteristics, ACS characteristics, and perceptions and knowledge were all associated with ambulance use in ACS. The fact that knowledge increases ambulance use and the need for behavioral change pose a challenge for health-care professionals.

    Abstract [en]

    BACKGROUND: Not much is known about the patients' decision time in acute coronary syndrome (ACS). The aim of the survey was therefore to describe patients' decision time and factors associated with this parameter in ACS. METHODS: We conducted a national survey comprising intensive cardiac care units at 11 hospitals in Sweden in which patients with ACS diagnosis and symptoms onset outside hospital participated. Main outcome measures were patients' decision time and factors associated with patients' decision time. RESULTS: In all, 1939 patients took part in the survey. The major factors associated with a shorter patient decision time were: 1) ST-elevation ACS, 2) associated symptoms such as vertigo or near syncope, 3) interpreting the symptoms as cardiac in origin, 4) pain appearing suddenly and reaching a maximum within minutes, 5) having knowledge of the importance of quickly seeking medical care and 6) experiencing the symptoms as frightening. The following aspects of the disease were associated with a longer decision time: 1) pain was localised in the back and 2) symptom onset at home when alone. CONCLUSION: A number of factors, including the type of ACS, the type and localisation of symptoms, the place where symptoms occurred, patients' interpretation of symptoms and knowledge were all associated with patients' decision time in connection with ACS.

    BACKGROUND:

    Various efforts to reduce patient delay in acute coronary syndrome (ACS) have had limited success. One reason might be a misinterpretation of the symptoms of ACS. The aim of this study was therefore to explore the characteristics and severity of symptoms among patients with an ACS in overall terms and in relation to the type of ACS, sex, age, and diabetes.

    METHODS:

    A total of 1939 patients at 11 hospitals in Sweden answered a questionnaire containing questions relating to the localization and intensity of symptoms, the presence of associated symptoms, the characteristics and experience of pain/symptoms, and the type of symptom onset.

    RESULTS:

    Patients with ST elevation differed from those without by more frequently having associated symptoms. They had higher pain/discomfort intensity and more frequently had pain with abrupt onset reaching maximum intensity within minutes. However, this type of symptom onset was only seen in less than half the patients with ST elevation and only 1 in 5 fulfilled all the criteria usually associated with a severe heart attack. Women differed from men in a few respects. They more frequently reported pain/discomfort in the neck or jaw and back, vomiting, and scored their pain/discomfort slightly higher than men. Differences between age groups were minor and there was no difference between patients with and without diabetes.

    CONCLUSIONS:

    The most striking finding was the low proportion of patients with the type of symptoms that are commonly associated with ACS. This is important for the planning of educational campaigns/programs to reduce patient delay.

    Abstract [en]

    Clinical features have low predictive value for the outcome of eating disorders. This study focuses on the possible predictive value of gender ideals. Data was from adolescent girls thoroughly assessed both at presentation and at a three-year follow up. Few variables differed between the outcome groups. However, perceptions of gender ideals, as measured by the Three Questions of Ideals, were found to contribute to the prediction of outcome. A discriminant analysis showed that gender ideals, maturity fears, sexual debut, and vomiting were significant discriminators in a set of factors, which correctly classified the outcome for 75.3% of the sample.

    Abstract [en]

    Three open-ended questions based on Steiner-Adair (1986) were used to study gender concepts in relation to ideals in adolescent boys and girls and eating-disordered girls. The answers were coded according to themes emanating from the collected data and not predetermined. Dichotomous differences were found between the genders regarding perceived ideals of body size, but clinical and nonclinical girls perceived ideals of society and media similarly. Individual gender concepts differed substantially from the perceived ideals, and were most often focused on lifestyle and life conditions. However, the clinical girls were somewhat more likely to emphasize appearance and loving relationships.

    Abstract [en]

    We report on first experiences from linguistic analyses of patient instructions from 19,8404 actual drug prescriptions regarding seven pharmaceutical products frequently prescribed in Sweden. The analysis includes expressions for amount, dose unit, dose interval, mode of administration, purpose and a few further details. Even simple processing seems useful to extract information from these short, rather formal text strings. We estimate the potential for calculation of prescribed dose from this material, and collected material gives a good starting point for more advanced linguistic analyses.

    The aim of the paper is to discuss the ability of the narrative perspective to explain change, and especially absence of change, in a group’s way of telling its story. The empirical case is the Filadelfia Church in Stockholm. In the discussion, I will particularly emphasize the need to interpret the story in relation to context but also in relation to the group’s norms and goals.

    The aim of the thesis is to examine how a group’s self-image is created, sustained and if necessary changed during the revolutionary 20th century. The study takes its point of  eparture in the idea that a self-image is essential for an organisation’s cohesion and collective identity. The study object is the Pentecostal Philadelphia church in, Sweden, established in 1910. In concrete terms, the thesis examines the self-image of the church as it is expressed in its narratives and activities from 1910 to 1980. The lf-image is analysed with the aid of the concepts ‘boundaries’ and ‘symbols’ and in relation to social and organisational change processes. In the main the empirical material  onsists of official documents such as jubilee publications, annual reports and the weekly newspaper Evangelii Härold.

    The main contribution of the thesis is an analysis of the church’s historical narrative. In this narrative the overall theme appears to be the small and faithful group that due to God’s influence and despite opposition became a major  nd significant church -- a success story. The theme also remains the same when the circumstances change. Narrative theory emphasises that the narrative has to be changeable order to be serviceable. However, my study shows that the narration, at least at the level highlighted in the thesis, is inert. The self-image seems to be difficult to change but is not necessarily static. The narrative is shown to include strategies for dealing with internal change processes and changes in society.

    Studier om väckelserörelsens rumslighet i form av lokaler och byggnader verkar till övervägande del intressera sig för arkitekturhistoriska kartläggningar eller analyser där lokalernas utformning och dess inre utsmyckning är i fokus. Det är främst historiska och sociologiska men även teologiska orsaker som lyfts fram i dessa studier. Däremot saknas aktörernas perspektiv på de egna lokalerna.

         Pappret fokuserar därför hur aktörerna, de som använde lokalerna, beskrev sina egna lokaler och hur dessa berättades in i aktörens egen historia och därigenom blev en del av deras kollektiva identitet. Detta perspektiv är delvis inspirerat av den teoretiska diskussion om plats som förekommer inom kulturgeografin. Platser diskuteras där inte enbart som något statiskt och fysiskt utan som en pågående tillblivelse genom det fysiska rummets möte med mänskliga praktiker och de tankar, idéer och känslor som knyts till ett specifikt rum.

    Abstract [en]

    Studies of the Christian movements’ spatiality in terms of premises and buildings seems mainly to be focus upon architectural history surveys or analyses of layout of rooms and the interior decoration. Historical and sociological as well as theological reasons are primarily highlighted in these studies. However, they seem to lack the agents’ perspectives on the premises.

    The paper focuses on how the agents used and described their own premises and other places (the city's physical dimension) and how they were made a part of the agents’ group identity. This perspective is partly inspired by the theoretical discussion about place that exists in Human geography. Within that discipline place is discussed as an on-­‐ going creation through conjunction of the physical space with human practices, ideas, strategies and emotions.

    The aim of the paper is to shed light on the above-­‐mentioned topic by studying the emerging Swedish Pentecostal revival and especially the Filadelfia Church’s in Stockholm premises and use of place during the early 1900s. The study is based on annual reports, the congregation’s weekly and jubilee publications.

    Konflikter är något alla människor ingår i, och för att lösa dem på bästa sätt krävs kunskaper och förmågor i konflikthantering. Redan i förskolan behöver barn få förutsättningar att utvecklas i konflikthantering, och där har pedagoger en betydelsefull roll. Syftet med denna studie är således att belysa pedagogers handlande i barns konflikter i förskolan samt hur handlandet kan främja barns förutsättningar att utveckla förmågor i konflikthantering. Studien är kvalitativ, utgår från konstruktionism och hermeneutik, och har en induktiv ansats. Datainsamlingsmetoden är observation. Resultatet visar att pedagoger inträder i barns konflikter dels utifrån uppmärksammande av barns verbala och ickeverbala förfrågningar, och dels på pedagogers eget initiativ. Datainsamlingen visade dessutom att pedagoger löser konflikter för respektive med barn. Studien visar att pedagogers handlande både kan främja och begränsa barns förutsättningar att utveckla förmågor i konflikthantering. För att främja utvecklingen bör pedagoger lösa konflikter tillsammans med barn, i de situationer som det finns utrymme i förskolans verksamhet.

    This article provides directions for advancing the conceptualization of the relationship between social movements and institutionalization, based on a case study of the Swedish environmental movement strategies. We argue that the concepts of (de) responsibilization and (de) politicization provide tools for an improved analysis of the dynamics of how social movements interact both with established political institutions and corporations in a new context. The introduction of new regulatory frameworks in environmental politics has shaped interaction between social movements and the state in new ways, involving neoliberal responsibilization, meaning active involvement by civil society and business in political responsibilities previously associated with state agencies - a development involving an increasing emphasis on market mechanisms. We argue that this has involved a de-politicization of environmental issues in the sense that it engages political actors in a moral discourse and a technocratic practice that suppresses the (potential) articulation of social conflict through consensus building. However, we also show how movement actors resist the discourse that encourages them to take on certain responsibilities, thus engaging in a politics of responsibility. Empirically, we demonstrate how the changing strategies of the Swedish environmental movement in the 2000s need to be understood in relation to the following processes, indicating that the Swedish case has a general relevance for an understanding of the contemporary environmental movement globally: (1) the transformation of the Swedish model of welfare capitalism under the influence of neoliberal discourse; (2) international environmental policy developments, most importantly the emergence of climate change as a dominant issue globally.

    Bakgrund

    Djup sedering är associerat med förlängd vårdtid och ett flertal komplikationer för intensivvårdspatienten. Även för ytlig sedering medför komplikationer, stress och lidande. Studier visar på vikten av adekvat sedering, samt att skalor och protokoll används. Syftet med denna studie var att beskriva sederingsnivå, andel optimalt, över- och undersederade patienter, vårdtid i ventilator och vårdtid inom intensivvården för patienter på en svensk intensivvårdsavdelning.

    Metod

    Studien, med deskriptiv design, inkluderade konsekutivt sederade, ventilatorbehandlade patienter, 18 år och äldre, på en svensk intensivvårdsavdelning under nio månader. Primära utfallsvariabler var sederingsnivå, utifrån Motor Activity Assessment Scale och andel optimalt, över- och undersederade patienter. Sekundära variabler var vårdtid i ventilator och vårdtid på intensivvårdsavdelningen.

    Resultat

    Studien omfattade 34 patienter. Under vårdtid i ventilator med endotrackealtub hade 28  patienterna optimal sedering mer än två tredjedelar av vårdtiden. Medianvärdet för sederingsnivå under vårdtid med endotrackealtub var MAAS 2 (kvartilavstånd 2-2,25) jämfört med medianvärdet för sederingsnivån med trackealkanyl som var MAAS 3 (kvartilavstånd 2-3) (P < 0,001). Trettio patienter (88,2 %) var optimalt sederade med ett MAAS-värde på 2-3 (md) sett till hela vårdtiden. Fyra patienter (11.8 %) var översederade med ett MAAS-värde på 0-1,5 (md). Ingen patient var undersederad sett till hela vårdtiden.

    Konklusion

    Studien visar att majoriteten av intensivvårdspatienterna var optimalt sederade under större delen av vårdtiden i ventilator. Endast ett fåtal patienter var översederade och ingen patient var undersederad större delen av vårdtiden. Patienter vårdade med trackealkanyl var ytligare sederade än de med endotrackealtub.
