Abstract [sv]
Mobile health, eller mHealth, är ett begrepp som används för att beskriva vård och folkhälsoinsatser som ges med stöd av mobila e-hälsotjänster. Sjukvårdssystem över hela världen använder i allt högre utsträckning mHealth som ett komplement till traditionell vård. Snabba tekniska framsteg i utveckling av mobila tjänster och i utbyggnaden av mobila nätverk skapar möjligheter att integrera mHealth med existerande e-hälsolösningar, t ex nuvarande journalsystem. I Sydafrika har primärvården förstärkts med teams av så kallade community health workers (CHW), och i allt högre utsträckning utvecklas nu mobila lösningar för att stötta denna nya yrkeskategori. I Sverige har IT använts länge inom hälso- och sjukvården, men bara en bråkdel av de möjligheter som mHealth erbjuder har implementerats i primärvården. Fler och mer djupgående utvärderingar av mHealth krävs för att kunna uppnå de högt ställda målen i nuvarande nationella e-hälsostrategier. Målet med denna studie är att utvärdera olika aspekter av mobila IT-system som används av CHW och vårdpersonal i landsbygdens hemsjukvård, i både Sydafrika och Sverige. Utvärderingen kommer att inkludera de effekter dessa mobila lösningar har på specifika indikatorer relaterade t ex till vårdkvalitet, arbetsprocesser och användbarhet. Förväntade resultat: En jämförelse mellan implementering av mHealth i Sydafrika och i Sverige förväntas ge användbara insikter i hur mobila lösningar kan förbättra vårdprocesser och hälsa i olika kontexter.Studien kommer att använda sig av en socio-tekniskt modell, och resultaten analyseras utifrån 8 dimensioner som identifierats i detta ramverk. Lärdomar från studien kommer att ge grund åt vidare arbete vad gäller uppföljning och utvärdering av hur mHealth används av vårdpersonal och CHWs. Baserat på resultaten från både Sydafrika och Sverige kommer ett ramverk för utvärdering av mobila IT-lösningar i hemsjukvård att designas.
Abstract [en]
Mobile health, or mHealth, is a term used for the practice of medicine and public health supported by portable mobile communication technologies. Health care systems around the globe are increasingly incorporating mHealth as a complementary strategy for strengthening health care systems. Rapid advances in mobile technologies and applications and the continued growth in coverage of mobile cellular networks provide opportunities for the integration of mobile health into existing eHealth services, thus transforming health care delivery. As part of Primary health care (PHC), South Africa initiated outreach teams that include community health workers (CHWs) to strengthen the health care system. Increasingly, mHealth solutions are being developed, tested, and piloted with CHWs. In Sweden, although IT has been used in health care for decades still only a fraction of the potential of modern information technologies is used in PHC. Therefore, the evaluation of mHealth is central to the success of national eHealth strategies for efficient, effective and quality health care. The aim of this research is to evaluate different aspects of mobile IT-systems used by CHW and healthcare professionals in rural home care, in both SA and Sweden. The evaluation will include the impact of these mHealth technologies as used by CHW and health care professionals on specific health care indicators.A comparison of SA and Sweden mHealth will provide useful insights into how mobile technologies could be used to improve health care delivery and health outcomes in different contexts.The study will utilize a socio-technical evaluation framework and report on the eight dimensions of the health care technology. Lessons learned from this study will provide grounds for monitoring and evaluation of the use of mHealth by health professionals and CHWs. An evaluation framework of mobile IT systems in health care will be designed based on a number of evaluation studies in both SA and Sweden.
