Abstract [sv]
Urinvägsinfektioner (UVI) är bland de vanligaste bakteriella infektionerna och kräver snabba och tillförlitliga diagnostiska metoder, särskilt i takt med ökad antibiotikaresistens. Denna studie utvärderade automatiserad direkt resistensbestämning av Enterobacterales från urinprover och jämförde resultaten med EUCAST:s standardiserade diskdiffusionsmetod.
Totalt analyserades 216 bakteriestammar från urinodlingar: Escherichia coli (n=173), Klebsiella spp. (n=22), Proteus mirabilis (n=8), Citrobacter spp. (n=8), Enterobacter cloacae (n=3) och Serratia spp. (n=2). För att öka antalet stammar med resistens mot mecillinamresistens inkluderades dessutom sex frysta, resistenta isolat. Testade antibiotika var nitrofurantoin, mecillinam, trimetoprim, cefotaxim, ciprofloxacin och ceftazidim. Båda direkt resistens- och standardiserad diskdiffusionmetod tillämpades på samtliga isolat, och hämningszonerna avlästes samt kategoriserades i S, I, R för respektive antibiotika.
Överensstämmelsen var 98 %, med 16 avvikelser (1,5 %): 3 major errors (0,3 %), 13 minor errors (1,2 %) och inga Very Major Error identifierades. Zonstorlekar inom S-kategorin för ciprofloxacin och ceftazidim visade en statistiskt signifikant skillnad för E.coli. Resultaten tyder på att direktresistensmetoden kan fungera som ett effektivt verktyg för snabb antibiotikaresistensbedömning av Enterobacterales vid urinvägsinfektioner. Även om metoden visade god överensstämmelse för dessa prover, krävs ytterligare studier för att bedöma dess tillförlitlighet i andra sammanhang.
Abstract [en]
Urinary tract infections (UTIs) are among the most common bacterial infections, requiring rapid and reliable diagnostic methods, especially with rising antibiotic resistance. This study evaluated automated direct antimicrobial susceptibility testing of Enterobacterales from urine samples and compared it with EUCAST’s standardized disk diffusion method.
A total of 216 bacterial isolates from urine cultures were analyzed: Escherichia coli (n=173), Klebsiella spp. (n=22), Proteus mirabilis (n=8), Citrobacter spp. (n=8), Enterobacter cloacae (n=3), and Serratia spp. (n=2). To increase the number of strains with mecillinam resistance, six frozen resistant isolates were also included. The antibiotics tested were nitrofurantoin, mecillinam, trimethoprim, cefotaxime, ciprofloxacin, and ceftazidime. Both direct susceptibility testing and the standardized disk diffusion method were applied to all isolates, and inhibition zones were categorized as S, I, R for each antibiotic.
The agreement between the two methods was 98%, with 16 discrepancies (1.5 %): 3 major errors (0.3 %), 13 minor errors (1.2 %), and no very major errors identified. Zone diameters within the S category for ciprofloxacin and ceftazidime showed a statistically significant difference for E.coli. The results suggest that the direct method can serve as an effective tool for rapid resistance assessment in Enterobacterales UTIs. Further studies are needed to confirm its reliability in other contexts.
